What insurance companies are offering annuities or life insurance products that can be used in a 412(i) defined benefit plan?

I am aware of the following companies active in this market.
American National, Berkshire Life, CUNA Mutual, General American, Lafayette Life, Massachusetts Mutual, National Life of Vermont, and Northwestern Mutual.

Conseco and Lafayette are probably the most experienced.
