Are you required to have a plan sponsor signature on a hardship withdrawal form as authorization to process the distribution?

What are the procedures for regular distributions from the plan?

Even if they are not required, I would think it would make sense for the PA to sign off on the hardship, because it is the PA who should determine if the h'ship is valid.
