Can funds from a SIMPLE 401(k) be rolled over into a SIMPLE IRA?

Thanks, I checked the attached rollover chart this morning before I posted, but SIMPLE 401(k) is not included. Is a SIMPLE 401(k) a qualified plan, a SIMPLE IRA or something else for rollover purposes?

My understanding is that a simple 401k is a qualified plan.

No. A simple 401(k) can not be rolled over to a SIMPLE-IRA. No rollover to a SIMPLE IRA is permitted from a qualified plan of any type.
