The instructions are not very clear on this.
I seem to recall that it does not have to be filed/attached if total fees paid to all service providers is less than $5,000
Help!

Line 2 of Part I must be completed to report contract administrators and persons receiving, directly or indirectly, $5,000 or more in compensation for all services rendered during the year.
Form5500Guru™
Plans with 100 or more participants filing Form 5500 are not required to file a Schedule C if they have not paid fees, directly or indirectly, over $5,000 to a service provider.

Be careful of the indirect fees!
