Does anybody know off-hand - where is the model languagy to be used to draft a QDRO? Thank you

One sample is in the ASPA study guide for C-1 (1998) though it might be in earlier editions as well

The official source for language is IRS Notice 97-11. If I remember right, a copy of that notice is in the DoLs QDRO booklet, which is posted at their website. (www.dol.gov/dol/pwba). Look under the publications heading.
