When DOL Reg. 2590.606-1(d) & (f) permit a single notice to covered employee, spouse and dependents, how are people addressing these letters?

Depending upon who is covered, I will send it as:
Joe & Nancy Employee
Joe & Nancy Employee & Covered Dependents
Joe Employee & Covered Dependents
