Is anyone familiar with Hawaii's speciific requirements for medical plans?

Hawaii, is exempt from the ERISA requirements, by statute. They operate under a single payor system. Blue of Hawaii. There are procedures for an out of state employer to qualify their out of state insurance. Usually not worth the effort. Also the rules require some coverage for part time EEs.

THey also have Kaiser and mandate certain benefits such as massage (great place to live if you are masseuse therapist I would imagine). They mandate that employers pay for family and also mandate that employers have group insurance - which is not required by ERISA. You should probably do a search on the department of insurance website for more details. Kaiser and Blues of Hawaii as far as I know.

Are you sure that they have a single payor system? I don't see how they could since aside from BCBS (HMSA) and Kaiser about 10% of the market is covered by a number of other carriers.

If you are an employer in Hawaii, you must make coverage available to employees who work at least 20 hours per week for at least four consecutive weeks. There are mandated and minimmum benefits and other issues. The employer must contribute at least 50% of the employee premium.

There are cetain employers, mainly agricultural, whose seasonal employees can be excluded.
