How long is a spousal consent valid?
Does the spouse's signature on a distribution become stale after 90 days as does a participant's signature?

It depends on what the spouse consented to -- the terms of the acknowledgements, consent, etc. See Regulations Sec. 1.401(a)-20 Q&A 31© allowing general consents.
