I am looking for a copy of a self-funded medical plan document.

Two possible resources:
1. Call a health insurance company and ask to see a copy of a sample document.
2. Call a Third Party Administrator and ask to see a sample copy.

You may also want to call some of the HR people at other companies in the area and ask them if yo can have a copy of their medical plan document.

Better specify whether you're looking for completely self-funded, or self-funded with a stop-loss component.
Kirk Maldonado
I disagree with NAC. Whether or not there is stop-loss insurance should not affect the terms of the plan document. However, the SPD needs to disclose the funding of the benefits.

Bill:
What I'd Like to know is have you found what you were looking for? If so, where did you get it?
