Can an emloyer offer a match on contributions exceeding 100%? For example, 200% match on the first 5% contributed?

It would depend on your plan document (discretionary?). You would also need to pass the ACP test.

Yes, although it isn't very common, one can have matching contributions greater than 100% of the deferrals that are being matched.

And don't forget to watch the 404 and 415 limits on large matches.
