Has anyone seen a sample 401(a) that is written specifically as a governmental plan without all the ERISA language? I have the list of what is applicable and what is not, but it would be nice to see it all in one document.

You might try looking at the statutory language setting up the Public Employees Retirement System of Idaho. It is one of the few (if not the only) state system to obtain regular IRS determination letters. Thus, it is likely to be at least reasonably up to date in terms of compliance with federal law.
